P-Graph screens your healthcare staff against the federal exclusion lists, and any state Medicaid lists you add. It re-screens the day a list changes — not once a month — and remembers every decision you have already made.
Rule on a possible match and it stays ruled — it is not raised again, and the next report shows what changed rather than the same queue. Tools without memory hand you the same ambiguous names every cycle, and that repeat review is most of what screening actually costs you.
A match on National Provider Identifier is reported as confirmed. A match on name and date of birth — often the only evidence available for aides, nurses and billing staff, who carry no NPI — is reported as requiring your verification, never as a finding. You are never told someone is excluded on the strength of a name.
Every person on the roster appears in the report, not only the matches, with what was found for them, which list it came from, and when that list was published. Proving you screened the other four hundred people is the part an audit turns on.
